Your assignment is to write an assessment with a diagnosis, a case conceptualization using one of the four theories we studied in this class (Narrative, CBT, Solution Focused, or Psychodynamic) and a treatment plan based on the case below. Remember, this is a theory-based case conceptualization and treatment plan, so your conceptualization and treatment plan should be congruent. For example, if you conceptualize the case using CBT you should have a CBT treatment plan. The report should be written in narrative style. That means you should use complete sentences and do not use bullet points or incomplete sentences. You will be evaluated based on how well you complete the report. It should be done according to common professional standards that we have discussed and are contained in the practicum check list on Sakai in resources and it is posted below. You have to use your imagination to complete the mental status exam and transference-countertransference sections of the report and that is fine. It is recommended that you spend a good amount of time completing this report and reviewing it several times before you turn it in. You should use the review function in Word to edit the report before submission. If words are underlined that is Word telling you a correction may be necessary. This should be your best effort based on what we have studied in class.
