CC: Experiencing foul-smelling odor with a burning sensation.
HPI: Pt is a 30-year-old AA female named who presented with her mom to the clinic has recently experienced some discomfort in her urinary tract recently. The patient complains about dysuria – pain while urinating, which started two weeks ago. She experiences a burning sensation every time she urinates and pain with sex. She also notes that the frequency of urinating has also increased. She feels a sudden urge to urinate. PT also notes that her urine has a foul smell and a thick whitish discharge. while also noting that she experiences some pain above her pubic bone. Relatedly, she notes that she is sexually active, and she had sex with but it was protected, feels no fever, chills, flank pain, and with a history of substance use. However, she is physically active by being on a gymnastics team in her school. PT notes that she has not experienced anything that worsens or improves the pain. The patient scores the pain at three on a scale from 0-10, implying its mildness.
