This week, you will choose one of the scenarios below and apply a clinical interview with the fictional client case. Develop 3-5 questions for each major area you will assess with the client (not necessary for demographics). Be sure to include the following major areas/headings in your clinical interview write-up:
Demographics (no need for questions, you can just list identifying information)
Identity/Diversity Aspects (this section should attend to gender identity, race, ethnicity, ability, SES, religion, sexual orientation)
Presenting Problem (what has brought this client into treatment)
Substance Use (this section should attend to substance use history, patterns of use related to frequency/setting/duration, possible impairments)
Relevant Psychosocial History (this can include family history of substance use issues, criminal/legal history, educational/occupational history, etc.)
Mental Health Functioning (this section can include relevant aspects of mental status, emotional presentation, important thoughts/attitudes/beliefs)
Social Functioning (this section can include social supports, relationship history, etc.)
Medical Issues
You will provide answers to each of the questions you list in your clinical interview form. This will give you a sample of how your clinical interview questions produce results and findings that can then inform the case conceptualization process.
Scenario 1: Serena is a 24-year-old Latina, first-generation college, female graduate student at the local state university. She was pulled over after leaving a restaurant at 10 p.m. and swerving in traffic. She was arrested and charged with a DUI. She blew a .18 on the breathalyzer and appeared to be significantly impaired at the scene. She comes to your office on Monday after being in jail for the weekend and just making bail that morning. She is still wearing her clothes from Friday night, and it looks like she was dressed for an evening out. She reports that she and her friends were celebrating happy hour, which lasted most of the evening, and planned on meeting them out for more drinking when she was pulled over. Her friends did not drive; they took a cab to the next place where they were to meet up. Serena has not told them what happened. She appears embarrassed and cries while you are meeting with her. She has never had any issues with the law before. She reports regular alcohol use while in school due to graduate study pressures but does not report any use of other substances. She used to smoke cigarettes until she couldn’t afford to do so any longer. She does not endorse drinking while in class or before class but does admit to drinking every night when she is studying, at least 1-3 beverages in an evening. She has a roommate, who bailed her out of jail, but the roommate is not a close friend and is unaware of the drinking issue as Serena drinks in her room alone while studying.
