List 15 questions that you as a social worker, working in a social service agency would need to ask a client in order to provide services to them. Consider what agency you would be working for, some examples would be; a homeless shelter, a hospital as a medical social worker, a home health social worker, a social worker at a domestic violence shelter, a social worker in a psychiatric hospital, a social worker at CPS, a school social worker .
The questions should be targeted to give the social worker information to be able to provide services. For example, if you were a social worker working at a housing assistance agency, you would need to ask questions in order to understand the kind of help the client would need.
Some examples of questions would be, how much do you pay in rent each month, do you work, how much money do you make each month, how many people live in your household, what are your monthly expenses, do you have running water and electricity, ect.
Develop your 15 questions using a Word document. Put your name at the top and state the fictional agency you are working for; example-XYZ Home Health, or XYZ Medical Center. This helps me understand the context of your questions. List your questions 1-15.

You will be interviewing a person of your choice and asking them your questions. You will let your interviewee know that they are to imagine themselves as a client at the agency where you work. The interviewee is not to answer questions from their own life, their answers will be fictional.You will print out your 15 questions prior to the interview and hand write the answers your interviewee gives you. Once you have completed your interview you will transfer the handwritten answers to typed into your 15 question Word document form. Your paper will include; your name, the fictional agency you are working for, your questions and the answers you were given by your interviewee. Finally, you will write your recommendations for services based on what your interviewee shared with you.Last, you will submit 2 papers on October 6th. The first will be your 15 questions with your handwritten responses from your interviewee.
