Case Study

John
John Davies is a 17-year-old year 12 student diagnosed with acute myloid lymphoma. He begins Chemotherapy that will hopefully bring the disease into remission. John’s father is a doctor and decides he does not want his son to know he has a life threatening illness. John becomes very ill as a result of the disease and chemotherapy, and you develop a good rapport with him during this time. The chemotherapy decreases the size of the lymphoma and John is discharged. He is to return to the ward weekly for a chemotherapy treatment and the following morning for a follow–up drug to stop the action of the chemotherapy drug on normal cells. John comes for his follow-up drug but very late in the day. He is putting himself at risk from the chemotherapy drug because of his lateness in receiving the follow- up drug. He states that he comes late because he plays guitar in a band and gets to bed very late. The nurses are concerned that because John does not understand the seriousness of his illness nor the seriousness of the drugs that he will continue to put himself at risk.
The nurses ask the doctor if a family conference could be called to discuss the issues of John’s on going care. Use the ethical decision making framework to determine your contribution to the case conference and your recommendation for his ongoing care.
