This assessment builds on the work you have completed for your forum posts in weeks 1-3 (part 1 of this assignment). It requires you to create a well-justified learning contract that will enable you to explore a particular idea, issue or practice problem relevant to your professional development, your workplace, or community, or contribute to the scholarship and development of your professional discipline or your discipline’s contribution to addressing health needs and priorities.

Your paper should include:

1.A title page.

2. A well-defined question/or focus statement to guide your project.

3. A clearly articulated practice problem statement (approximately 100-150 words).

4. A statement of significance, depending on the focus of your project. This might include significance to your professional development, workplace, community, profession or scholarship of the discipline to which you belong (approximately 100- 150 words).

5. A focused literature review exploring current research or other evidence/ authoritative information related to your question/or focus statement (no more than 500-600 words).

6. A completed learning contract template that clearly and succinctly compares your current abilities with your desired abilities, and outlines the best strategy/ies for bridging the gap in order to accomplish a self-determined and well justified project (points 1-4). The learning contract includes learning objectives, available resources, obstacles and solutions, deadlines, and measurements. Use the template provided. Use bullet points to complete (approximately 500-600 words).

7. A justification of the strategy/ies you will use to achieve your learning contract. This needs to be supported with reference to authoritative literature (approximately. 150-200 words).

PRACTICE PROBLEM

This project will focus on the practice problem of poor wound management and outcomes.

STATEMENT OF SIGNIFICANCE

The occurrence and the inability to definitely heal wounds effects the patient’s quality of life, is a significant financial burden on the healthcare system and an indication on the quality of care provided. A team approach to wound care management is not a new concept, but currently not used within the author’s current workplace environment. The practice is to employ individual wound care clinicians to manage patient’s wounds. This places substantial responsibilities on the individual clinicians and fails to recognize that no individual healthcare clinician is able to meet all the needs of the patient.
