Assessment and diagnosis of substance use is usually a daunting task. Engulfed by several barricades, the professionals in charge, and the patients involved in these processes have to overcome various challenges. Atop the barrier list is stigma and discrimination. Globally, the persistent nature of stigma towards substance use is overwhelming. Defined as the prejudicial and stereotypical attitudes people hold towards other people or certain traits, stigma causes people to distance, reject, and somewhat fear people suffering from substance use (Institute of Medicine, 2002). As a defensive mechanism, the patients become unwilling to seek medical support for fear of being treated as outcasts.
Finances are critical when seeking A-level medical treatment. Most nations have underdeveloped public medical facilities. Consequently, people have to resort to private hospitals for treatment. Due to their expensive nature, most patients cannot afford privatized medical facilities thus may opt-out of treatment procedures. Mental health systems can also act as barriers to treatment and diagnosis procedures. Today, medical organizations are fragmented. Patients have to wait for long periods to receive healthcare services (Institute of Medicine, 2002). It is no surprise to see clinics and hospitals understaffed yet overflowing with patients. Sections of these facilities lack the appropriate linkages to carefully track the treatment of a patient throughout and sometimes patient data is lost midway diagnosis or assessment. The disorganization and long waiting periods put off most patients and increase the developmental timeframe for the disorders as the patients await assessments and diagnosis.
The Most effective Treatment Setting
The treatment setting is determined by the severity and clinical recommendation. Having a patient undergo one setting throughout the treatment process would be ill-advised. As the patient progresses, they are moved from the intense units into lesser severity levels (Stevens & Smith, 2004). The transition between these levels is key as it allows the patient to acclimatize to normal life gradually. Level 1 patients can receive treatment with minor supervision and cannot be treated as level 3 patients who have to be clinically managed. The effectiveness of a treatment plan is thus determined by the severity of the patient use and no particular setting can be described as better than the other.

Substance abuse treatment is delivered in two basic settings or environments: inpatient and outpatient. Although the two types of settings vary widely by cost, recent evaluation studies have not found that treatment setting correlates strongly with a successful outcome. In fact, research has not found a clear relationship between treatment setting and the amounts or types of services offered, although there is a correlation between the services provided and posttreatment outcomes.
Initially, however, it is important to match the patient’s needs to a treatment setting. The goal is to place patients in the least restrictive environment that is still safe and effective and then move them along a continuum of care as they demonstrate the capacity and motivation to cooperate with treatment and no longer need a more structured setting or the types of services offered only in that environment. I believe inpatient hospitalization can be the most effective as it is the most intense.
Inpatient hospitalization includes around-the-clock treatment and supervision by a multidisciplinary staff that emphasizes medical management of detoxification or other medical and psychiatric crises, usually for a short period of time. Currently, hospital care is usually restricted to patients with (1) severe overdoses and serious respiratory depression or coma; (2) severe withdrawal syndromes complicated by multiple drugs or a history of delirium tremens; (3) acute or chronic general medical conditions that could complicate withdrawal; (4) marked psychiatric comorbidity who are a danger to themselves or others; and (5) acute substance dependence and a history of nonresponse to other less intensive forms of treatment.

Chemical dependency is defined as abused and uncontrolled use of drugs or alcohol. This excessive use can affect a person’s mental health status and physical health. At the same time, the family and friend’s emotional health and physical safety are at risk. Laban identified nine elements necessary for an effective chemical dependency treatment plan. The first element is known as the type of plan; which has three parts. The first part is initial which is done in 24 hours. Master is the second stage, where different treatments are addressing the problem. The last part is an update which is a revision of the master part. The second element is the problem list. The problem uses assessment data and the client’s perceptions of the problem. Tangible evidence and data are used in the third aspect, which is known as Indicators. The fourth aspect is referred to as Long term goals. Meanwhile, Short term goals are located in the fifth aspect. Goals are defined as the action or events that need to occur in order to resolve the problems. Meanwhile, objectives are known as the specific behaviors that will help to achieve the goals. Objectives form the sixth element. Objectives are realistic and measurable. Also, objectives contain some behavioral statements of the desire state or condition. The seventh aspect is known as methods of interventions. These methods include specific behavioral interventions and tasks assigned to the client to fulfill their objectives. Short term goals and long term goals are part of the methods as well. The frequency of the services provided is the eight aspects. The last aspect includes the ongoing evaluation and requires signatures. These specific aspects show the client’s involvement and commitment.
