2-3. My interest in the topic and an example. Me being a bedside nurse, I have witnessed many medication errors. One example that made me open my eyes was when the nurse was taking care of two patients in the same room, and they both had similar first names. Transport came to take one of the patients to get a CT with contrast. The nurse told the transport to take the patient by the door(bed 1), but the patient by the window (bed 2) was the patient that was supposed to go for the CT. The patient in bed 1 had ESRD (End-stage renal disease). The patient in bed 1 received contrast dye, By the time the nurse figured out that the wrong patient went down for the test, it was too late. The nurse tried to flush the dye out of the patient’s system by giving her fluids. The patient died two days later from the complications of receiving the dye. The hospital also ended up getting sued by the patient family. When I precept new nurses to the hospital I stress the importance of using the 7 medication rights for each patient every time they get medication to help prevent medication errors.
