WATCH/READ “RCA-Example” LINK BEFORE DOING ASSIGNMENT
Root cause analysis is an important tool used in manufacturing. For this assignment, you will conduct a root cause investigation on an injury that occurred when an employee was struck by a falling object. Read and study attached scenario document for a descriiption of the event, including a time line constructed with input from the people involved. Also look for hints that will help you construct your cause and effect to find root cause(s) and potential solution.

Using the attached RCA template link attached below (must click on template link and it will download), complete an analysis of this event and provide recommendations to your management on how to prevent recurrence.

Use the process as shown in the tutorial video example to do the following:
1. Using the template, Define the problem (What? When? Where? Significance?) – refer to the scenario and look for details. You are free to “fill in” or “make up” details of the story as long as they do not contradict the scenario.
2. Using instructions in template, Create Cause/Effect Chart to show the cause/effect relationships in this scenario (USING METHODOLOGY SHOWN IN RCA-EXAMPLE VIDEOS). Submit your diagram as a separate document because the formatting and visability do not work well when inserting into the template provided.
3. Using instructions in template, Brainstorm solutions
Identify potential Solutions. Try to come up with at least 5 potential solutions. Note that you could (and should) propose more than one solution for the same cause (Example: If I forgot to take roll, you might propose to fire me, train me on taking roll, write a procedure for me, generate a checklist, provide an assistant to take roll, etc.)
4. Using instructions and table in template, Score each potential solution.
5. Select which solution(s) you would recommend to your management team for implementation – see template
Submit your filled out report/template in a Word document to the RCA dropbox link in the week’s module. If you fill out by hand, please scan all pages of template into ONE document but it’s ok to submit cause and effect as separate document. Use smartphone apps such as Scannable if you do not have access to a professional scanner.
This must be your OWN work.
Scoring Rubric:
2 pts – Define problem (on template)
4 pts – Cause & Effect diagram- this must be done in order to get any credit toward steps 3-5
3 pts – Brainstorm solutions for causes (Steps 3)and Scoring & selection of solutions (Step 4) using template. (no points will be awarded without submission of cause and effect diagram)
1 pt – Written report to management of findings and solutions (Step 5) using template. (no points will be awarded without submission of cause and effect diagram)
