Behavior Change Project – Regardless of how successful an activity is, there are always unexpected difficulties. This activity is an opportunity to look back over your behavior change project and determine how successful the change has been. It is also an opportunity to find areas that could have been improved upon. In other words, knowing what you know now, what would you do differently if you were to make this change again?
In at least 250 words total and using the question and answer format below, respond to the following questions below.

3. Provide activity logs, journal entries, calendars, or some other tracking method for
evidence of steps you have taken toward your goals.
• Provide all evidence of your activities, do not provide samples. You must demonstrate sustained activity in working toward your goal.
• You may provide this evidence in a separate document if necessary.
4. Discuss which stage of the transtheoretical model of behavior change you are now in
with regard to this behavior.
5. Assess your progress, both subjectively (how you feel you are doing),
and objectively (using a quantifiable measurement) by using the same measurement tool used in your Behavior Change Goals submission.
• The examples below are only there to provide ideas for your project. You are not
required to use them for your own assessments.
o Subjective Assessment Examples: What has been easier than you though?
What has been more difficult? Is this a change you would like to make permanent? Are there any changes you would like to make going forward? Elaborate on these and other questions as necessary.
o Objective Assessment Examples: Weight loss/gain; change in resting HR; strength test; endurance test; change in grades; money saved, etc.
6. Discuss how working toward these goals has impacted your health and wellness.
7. Address any comments, questions, suggestions made regarding your previous
submission.
