PICO QUESTION
In mechanically ventilated patients in an Intensive care unit does the use of chlorhexidine oral solution as opposed to standard oral care practices reduce the incidence of ventilator associated pneumonia
P = mechanically ventilated patients in an Intensive care unit
I = chlorhexidine oral solution
C = standard care practices]
O = reduce the incidence of ventilator associated pneumonia
The purpose of this assignment is to begin (or continue) your search for evidence to answer your PICO question using technology to mine (to search for) information while using bibliographic databases and then interpreting the credibility of the selected evidence.
You will learn how to construct a basic bibliographic search to find evidence to answer your PICO question.
You will use Boolean operators available with the bibliographic databases.
You will plan a keyword search based on a PICO question and conduct a search of a bibliographic database based on a PICO search.
Complete following:
Complete the exercise below and submit answers A-D.
Conduct a literature search using your PICO question variables; create a search history document using any format – the database tool, a screenshot, or a word document and submit.
Complete the Literature Summary Table
Actions
form and submit. Example of the Literature Summary Table: Table Research Evidence Example-SR.pdf
Actions
Create a Reference page with the articles from the Literature Summary Table and submit.
Some extra information
Once you have decided on what you need to know, you need to determine where to find the answer and what type of source would be the ideal or best available evidence. In chapter 9 you learned about different types of evidence sources, including systematic reviews. You also learned some important techniques for structuring a search of a bibliographic search. These are some of the key tips for searching:
Identify keywords for each element of the question and any controlled language that represent these keywords.
Controlled language or MeSH terms are words that have been tagged as a synonym for other words; e.g., dyspnea is the MeSH term for shortness of breath, breathlessness, etc. MeSH terms can be “exploded” in the search strategy to capture any type of citation related to the term or its synonyms.
When you see .mp attached to a term in some bibliographic databases like MEDLINe, it means that the term should be searched as a key term.
Use truncation to capture different forms of a word at the last common letter of a key word, e.g., nurs* for nurse, nurses, nursing.
Use wildcards to manage different spellings of a word, e.g., p$diatric for pediatric and paediatric.
Use the Boolean operator “OR” to broaden a search, e.g., link all keywords representing the population with OR.
Use the Boolean operator “AND” to narrow a search, e.g., link the lines of a search that represent all the keywords for each element of a question with AND.
Know where to search, e.g., bibliographic databases for individual studies and systematic reviews, www.guidelines.gov for clinical practice guidelines.
1. Exercises
Conduct the following search strategy, and answer the questions A – D that follow:
Explode pulmonary disease, chronic obstructive (this means it is MeSH).
chronic obstructive pulmonary disease.mp (.mp means search as keyword).
Combine 1 OR 2.
Explode arm.
Explode upper extremity.
upper extremity.mp
upper limb.mp
Combine 4 OR 5 OR 6 OR 7.
Explode exercise OR exercise therapy.
Explode dyspnea.
Combine 3 AND 8 AND 9 AND 10.
A. Line 9 means that both the exercise and exercise therapy must be found together in an article.
True False
B. Line 8 means that any article that is about arms, upper extremities, or upper limb or any article that means upper limb or upper extremity will be found.
True False
C. Line 11 means that dyspnea is a MeSH.
True False
D. Which line means that only articles that are about all of these terms and their synonyms will be found: COPD, upper extremity exercises, and dyspnea?
Line 3
Line 8
Line 10
Line 11
2. Literature Search History
Read this article EBP Inquiry in PubMed.pdf
Actions
and plan and save a search strategy for your PICO question.
Conduct the literature search using any FIU library database CINAHL, MedLine, etc. Create a search history using any format – the database tool, a screen shot, or a word document. You may need to try different searches in trial-and-error fashion.
Submit your Search History and answer the questions below [some answers are used in the Methods section of your literature review Paper #4].
How many hits did you get for the final line of your search?
What is your assessment of the effectiveness of your search? Are you confident that it was adequately sensitive and specific? Why or why not?
How might a librarian help you in your search? What would you ask a librarian if you needed help?
After conducting the above electronic search, identify five research studies related to your PICO question.
3. Complete the Literature Summary Table.docx
Actions
At least four of the studies must be QUANTITATIVE research studies. Only one study may be qualitative studies, but this is not required.
Summarize the five research articles on the Literature Summary Table which meet the following inclusion and exclusion criteria:
Original research studies published from 2017 to present from a peer reviewed publication.
At least two must be published in a nursing journal.
At least four of the five studies must be quantitative.
One study may be qualitative but not required.
Do not use review studies, systematic reviews, literature reviews, meta-analysis or meta-synthesis articles.
4. Submit a reference page with all the articles listed on the Table using 7th edition APA Manual formatting
