The clinical issue of interest I choose using PICOT framework: “Does sequential compression device (SCD) effective as a prophylactic device to prevent Deep Vein Thrombosis (DVT) on adult patients on extended length of stay in hospital; as compared to oral anticoagulants. The mnemonic-PICOT was introduced in 1995 by Richardson & et al, a framework for evidence based practice question (Davies, 2011).A common practice in hospitals is the use of of SCDs in combination with anticoagulants as a prophylaxis to prevent Venous Thromboembolism VTE. Despite minimal evidence in medically sick patients, SCDs are widely employed in hospitals in normal treatment (Dhakal et al., 2019). The PICOT acronyms addressed my question as follows: P- Patient-hospitalized adult patients, I-intervention- use of SCDs/oral anticoagulants, C-comparism- Oral anticoagulants, O-outcome-Prevent DVT, T-Extended long stay.
Database Search to support PICOT question
On Walden Library I searched keywords associated to my clinical question, words like “anticoagulants and sequential compression device” “Sequential compression and DVT” ,”DVT prevention and hospitalized patients”.I also used the Boolean as a guide in my search, limiting results to only Peer-reviewed articles, and choice of articles within last five years, phrase and terms were limited to relevancy of my PICOT question. The Database I used are Thoreau Multi-database search, CINAL database and Medline database. Thoreau Multi-database search resulted in many articles. This database searches across many other database. CINANL resulted to twenty four articles, with only scholarly-peer reviewed article the search result reduced to twenty articles, while MEDLine database returned twenty articles with Peer reviewed articles inclusive. One strategy I employ in my database search for an effective result is to formulate relevant clinical question, avoid use of abbreviations on Boolean and also choose database with multiple scholarly reviewed articles.
