In consultation with the clinical preceptor and faculty member, the student will choose an area for planned change in the clinical agency. The change may relate to clients, colleagues, or the system; for example, client care, staffing, policies, procedures, organizational patterns, evaluation, staff development, communication, etc.
This assignment will demonstrate your written/verbal communication and critical thinking competencies.
Please develop this assignment with careful attention to your communication skills, issue development and exploration, use of supporting literature, solution development, decision-making and reasoning process, research/literature integration, scholarship and format.
The change may relate to clients, colleagues, or the system; for example, client care, staffing, policies, procedures, organizational patterns, evaluation, staff development, communication, etc.
1. Choose a change theory, describe the theory, and discuss its applicability to your project. You will be graded on your ability to integrate the theory throughout your project.
•Identification of the problem
a. Describe the situation that poses a problem and why the problem represents
an opportunity for change.
b. Support the need for the planned change. This support may come in the form of literature sources, research, or an assessment process:
survey, questionnaire, interviews, statistical data, records, etc.
c. Analyze the potential for success of the planned change.
d. Include factors that you believe will enhance the achievement of the
identified change.
e. Include factors that you believe will detract from the achievement of the
identified change.
•The plan of action
a. Develop a plan of action
1)Consider various approaches/alternatives. You should have a minimum of 3 various approaches/alternatives to the problem. Each approach/alternative must have Literature support.
2)Determine consequences for each approach/alternative.
3)Prioritize each approach/alternative in rank order to determine which one is the
most likely to succeed.
b. Discuss how you plan to operationalize the approach selected.
c. Show specifically what changes you expect to occur as a result of your
planned change.
•Implementation of planned action
– Write this section in such detail that another person wishing to implement the same plan could do so using your plan as the guide.
a. Outline the plan for implementation of the change project.
b. Define responsibilities for implementing plan.
c. Describe action–what happened?
•Evaluation of consequences
a. Describe results. If you are not able to implement the plan, describe the
evaluation plan. This must detail all elements of the evaluation process.
b. Analyze effectiveness of plan.
c. Identify what facilitated the change.
d. Identify what inhibited the change.
e. Suggest alternative approaches, predicted consequences of alternatives
and probabilities for future use (What would you do differently next
time?)
