1.) NEEDS ASSESSMENT & BIBLIOGRAPHY
Fully research your topic & describe the need for your specific program based on the literature & Healthy People 2030 in detail and include the following:
Paragraph I: Write a description of the topic you selected from the topic selection assignment and who it impacts using at least 4 sources of data (i.e.., YRBS, County Health Rankings, Healthy people 2030, FYSAS, FL Health Charts, CDC, etc.)
Use the Public Health Data Websites resource posted on Canvas to find data on your topic and summarize in detail national, state, and local epidemiological data for your selected topic.
Paragraph II: Based on your findings, indicate a specific population (i.e., a particular health zone, specific race, gender, or age group in Duval County) with the greatest need based on the statistics.
Please describe any health disparities for your topic? (i.e., is it affecting more people of a certain race, ethnicity, gender, sexual orientation, age-group?)
Paragraph III: What are the main specific behaviors of the target population that are leading to the high numbers of the health issue (eating, smoking, exercise, etc.)?
From the literature, what are the major causes or reasons for the health behaviors, and what knowledge, attitudes & skills have been successful in changing those behaviors to improve this issue?
Paragraph IV: Find at least 6 peer-reviewed journal articles on your topic and summarize common themes about what interventions or programs have worked in previous research to address this health issue.What strategies have previous programs used that have had positive results for changing knowledge, attitudes, skills and behavior to positively influence your topic?
What is recommended in journal articles for future programs on this topic? (Look in the discussion section of your peer-reviewed journal articles for future recommendations)
Do not write an annotated bibliography for each article. You should find common themes and summarize as if you were telling a friend about what you learned from reading all of the articles, not each individual article.
Paragraph V: Draw conclusions from the needs assessment and intervention research on what can be done about this issue. The final sentence of the paper should be:
Fill in the () with your information: Based on these findings, the purpose of my program will be to (HP2030 objective) in (population (i.e. specific gender, race, and age group) by targeting (risk factors).
Formatting:
Double space and use 12-point font throughout.
Write in present or past tense to describe current or past data and research (3rd person)
Cite ALL facts/statistics, etc. that are not common knowledge!
Bibliography/References
Bibliography and citations must be in APA style & must be recent –preferably published within 5 years)
Each reference must be cited in the paper at least once.
The bibliography should include at least 4 sources for data on your topic and 6 peer-reviewed journal articles on your topic, and all 10 must be correctly cited within your paper
2.) GOALS & OBJECTIVES FIRST DRAFT (15 Pts)
This section should ONLY be 5 Statements
One Goal Statement (based on Phase 1 Social Assessment of the Precede Proceed Model)
Four Objective Statements: The four objectives will include:
One Process Objective (ie., # of classes held, “very good” rating on satisfaction survey for presentation, # of sites contacted for participation)
One Impact Learner Objective (based on Phase 3 Educational & Ecological Assessment)
One Impact Behavioral Objective (based on Phase 2b Epidemiological Assessment – Behavioral & Environmental)
One Outcome Objective (based on Phase 2a Epidemiological Assessment – Health Issue)
