Discussion: Case Studies of Legal Issues
Medical social workers play an integral role as support for patients and their families at the end of a patient’s life. Most physicians are not equipped to handle the emotional aspects at the end of life. Therefore, more often than not, medical social workers are the principal health care professional involved in educating, mediating, and coordinating end of life plans. In this capacity, medical social workers are charged with informing patients and their families about end-of-life decisions and providing counseling around these issues. Medical social workers must understand the terminology, technology, and legal issues that can dictate end-of-life care.
Consider the following cases:
End-of-Life Care: Case 1
Roger is a 43-year-old African American male diagnosed with HIV/AIDS and renal failure. He was recently admitted to the intensive care unit and put on a ventilator due to medical deterioration. The doctors inform his wife that there is no chance that his condition will improve and explain that he is being kept alive by the machines. Roger’s wife is distraught but knows that Roger did not want to be kept alive by a machine. The medical social worker goes to Roger’s chart and finds his medical power of attorney. Roger’s wife is listed as his agent; however, the document was not signed by two witnesses as required.
End-of-Life Care: Case 2
Margaret is a 72-year-old female with severe cardiovascular disease. She is admitted to the hospital after a stroke. There is no indication that she will recover and, if so, to what level of functioning. The doctors have informed her family that she is experiencing cardiac issues that might lead to heart failure. Should this happen, the doctors will have to take efforts to resuscitate her. The medical social worker and the doctor meet with the family to discuss a do not resuscitate order. The family is completely overwhelmed. They do not fully understand what the doctor is saying and they are in disagreement about what to do.
To prepare for this Discussion, review this week’s resources including the case studies. Select one of the cases. Think about the end-of-life issues including living wills or medical power of attorney.

Post an explanation of how you as a medical social worker would handle the case you selected. Explain the services you might offer to the family within the applicable laws and your scope of practice. Maria is a Puerto Rican woman who was diagnosed with breast cancer at age 35. During the three and half years that she lived after diagnosed, she received a variety of services from a cancer support organization, including weekly individual supportive counseling with a social worker and group therapy. At the time of her diagnosed, Maria had been married to Joseph for 13 years. They experienced fertility problems because of her cancer treatment. Maria reported to her Social Worker that this was a major source of conflict in the relationship and that she did not feel emotionally supported by Joseph.
Maria began working in a bank at age 17 and by age 30 had attained a middle management position. For two years after diagnosis, she worked full-time while receiving chemotherapy every three weeks, refusing to take time off for her cancer treatments. She reported significant depression, anger, and apprehension about recurrence. Within six months of diagnosed, she had begun to reexperience previous losses, most significantly the loss of her mother to breast cancer when Maria was 10 years of age. She reported consistent faith that the chemotherapy would cure her mother and hopeless betrayal by God when her mother died. Her oncology social worker suggested a referral to a psychiatrist for Maria’s worsening depression, which Maria declined, saying that she did not wish to take antidepressant medication.
Ultimately, Maria agreed to a stem cell transplant and left her job after a year of disability leave. She agreed to see a psychiatrist for a one-time consultation yet continue to decline antidepressant medication. The social worker reported that she became increasingly fatigued, depressed, and agoraphobic (e.g. she stopped going out alone secondary to panic attacks). Her cancer recurred after one year and she died 18 months later.
Answer these questions based on your interpretation of Maria’s case:

Based on the chapter you read AND Molly Bang, you will explore techniques used in creating an image and explore different types of media (be creative)- Should not be computer generated.
Select ONE book from your chosen collection (a reminder that the list of books is on page 1 of this module) In the commentary/analysis, you’ll list the name of the book, include a picture of the scene you chose, and the page number. Read the book. Look closely at the pictures and the text to determine the meaning of each scene. Look at expressions, body placement, colors, mediums, etc…
elect one image to re-illustrate. Pick any image that is interesting to you or one that represents a pivotal moment. Or, perhaps you want to select an image that you would feel comfortable drawing? Take a picture of that page with the page number to post to the DB
Fold a blank piece of paper into four squares.
Choose four types of media and illustrate the same scene four times. We want you to be creative with media. Go beyond crayon and pencils and select media that contributes meaning to the story. Go beyond the media featured in our example. Try Legos, collage, shapes (like Molly Bang), sticks, food, etc. Each square should feature a particular media (name the media or show it)!!
Take a picture of your final product and write commentary and analysis. When you look at the images, what do you notice? How does the media affect the message? How does artistic style affect the message? CITE- Use your textbook to help you discuss the illustrations (pp. 126-130). Embed your image (not attach or upload) AND the original image (scan or pic) then write commentary answering the questions above. See M1 about embedding images
Remember- Make sure you include the name of the book and page number of the original image with the original image.
Review Rubric- There should be a strong connection to the module readings in the commentary in order to get full points.
The image I chose was from “Happy To Be Nappy”, the four media types I chose were crayons (top left), paint (top right), beads (bottom left) and tissue paper (bottom right). When I look at the different medias I notice that they all portray the original picture differently, and each can interpret a different meaning. For instance with the crayons, we can clearly see that she is happy since she is smiling and sniffing her hair. Whereas, with the beads it is unclear that she is sniffing her hair, but still looks happy. Each person has a different perspective of what the message is conveying, which means they may recreate the picture completely different than what is being shown in the book itself, or from what the illustrator had in mind. On page 128, Dr.schnider discusses how to interpret a picture books message based on the illustrations which is important for this page of the picture book I have chosen. The author says “girlpie hair smells clean and sweet”, the illustration shows the girl smiling (due to her hair being clean and smelling her fresh clean hair), which is the main takeaway from this page. So, it is important that the nose and sniffing motion is defined through the media.(THIS IS AN EXAMPLE)
